Advance Medical Directive (AMD) is a document that you sign in advance to inform the doctor treating you (in the event you become terminally ill and unconscious) that you do not want any extraordinary life-sustaining treatment to be used to prolong your life. 1) ADVANCE MEDICAL DIRECTIVE

“……if I should suffer from a terminal illness and if I should become unconscious or incapable of exercising rational judgment so that I am unable to communicate my wishes to my doctor, no extraordinary life-sustaining treatment should be applied or given to me.”
2) ADVANCE RELIGIOUS DIRECTIVE
(PURE LAND)
“……when I face death, I hope that my closest relatives and friends will help me to die mindfully and attain birth in the Pure Land of Amitabha Buddha. There is nothing more important than to let me die mindfully by reciting the name of Amitabha Buddha……”
3) ADVANCE RELIGIOUS DIRECTIVE

“……when I face death, my closest relatives and friends will help me to die mindfully. There is nothing more important than to let me die mindfully by recollecting the Triple Gem…..”
